Foro SC Youth Soccer at The Highlands

In consideration of being permitted to participate in youth soccer practices, games, and related activities (the “Program”) organized by FORO Soccer Club and held at The Highlands School, 1451 E Northgate Dr, Irving, TX 75062, I, the undersigned parent or legal guardian, on behalf of the minor participant named in this registration, acknowledge and agree to the following:

1. Assumption of Risk. I understand that participation in soccer and related physical activities involves inherent risks of injury, including but not limited to sprains, fractures, concussions, and other injuries, and I voluntarily assume all such risks on behalf of the participant.

2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue FORO Soccer Club, The Highlands School, and their respective officers, directors, employees, coaches, volunteers, and agents (collectively, the “Released Parties”) from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury sustained by the participant while participating in the Program, whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

3. Medical Treatment. I authorize the Released Parties to secure and administer emergency medical treatment for the participant if needed, and I agree to be financially responsible for any costs associated with such treatment.

4. Photo/Media Release. I grant permission for the participant's image or likeness to be used in photographs or videos taken during the Program for promotional purposes, unless I notify FORO Soccer Club otherwise in writing.

5. Acknowledgment. I confirm that I have read this waiver in full, understand its terms, and am signing it voluntarily on behalf of the participant by checking the box below and submitting this form.
